This process is known as destatementisation. Below we guide you through how to fill the form, attach the right documents, and submit your request without delays.

Step by step guide to converting mutual fund units to demat form

The destatementisation request form is divided into four parts. You must fill each section correctly and courier it to us along with the supporting documents.

Section 1: Account holder details

Section 4: Distributor code and submission

If you want to change the distributor code during destatementisation, fill the Change of Distributor Code section and sign it.

Required documents

  • 2 signed DRFs per folio or ISIN
  • Self attested mutual fund statement or SOA
  • Self attested PAN copy
  • Affidavit if there is any name mismatch

Note: Once we receive the form, the update may take up to 5 working days. An acknowledgment will be sent via courier. After the request is processed by FYERS, the Registrar and Transfer Agent (RTA) may take up to 25 days to complete the update in their records.

You can share scanned copies of the required documents with [email protected] before sending the originals by courier to FYERS.
