At FYERS, we allow you to convert your mutual fund units from physical or statement form to demat format so that you can view, track, and manage all your holdings in one place. This process is known as destatementisation. Below we guide you through how to fill the form, attach the right documents, and submit your request without delays.

The destatementisation request form is divided into four parts. You must fill each section correctly and courier it to us along with the supporting documents.
Section 1: Account holder details

| Client ID | Your 8 digit FYERS Demat number |
| Holder Names | First, second, and third holders as per our FYERS records |
Section 2: MF scheme and folio details

| Folio No or ISIN | From your mutual fund statement |
| Scheme Name | The full name of the mutual fund scheme |
| Quantity | Mention in figures and also in words |
| Lock in Reason and Date | Tick Yes for ELSS schemes and enter expiry only if under lock in |
Section 3: Signatures

| Signature with RTA | Must match the signature with the mutual fund or RTA |
Section 4: Distributor code and submission
If you want to change the distributor code during destatementisation, fill the Change of Distributor Code section and sign it.

- 2 signed DRFs per folio or ISIN
- Self attested mutual fund statement or SOA
- Self attested PAN copy
- Affidavit if there is any name mismatch
