Step-by-step guide to filling the FYERS DRF form
The Dematerialisation Request Form is divided into four main sections. We recommend filling each part carefully to avoid processing delays or rejections.
Section 1: Account holder and security information

| DP ID | Enter 12089400 (fixed for all FYERS Demat accounts) |
| Client ID | Your 8 digit FYERS Demat number (view here) |
| Name of Holders | Names must match our FYERS Demat records (for all joint holders if applicable) |
| Name of the Company | As printed on the physical share certificate |
| ISIN | Found on BSE or NSE for the listed company |
| Quantity | Enter in figures and words |
| Number of Certificates | Mention in words (Example: "Two only") |
| Nature of Securities | Tick “Free Securities” or “Lock in Securities” |
| Lock in Reason or Date | Fill only if the shares are under lock in |
Section 2: Certificate and distinctive details

Section 3: Signatures and joint holder validation

Section 4: Submission checklist and courier address
- Two signed copies of the DRF
- Original share certificates
- Self attested PAN card copy
- Annexure if needed for extra certificates
- Affidavit or gazette copy if there is a name mismatch
