Download the Necessary Forms: Begin by downloading the forms here.
Fill in Essential Details: The form requires your DP ID, and your name as per your Demat account.
Sign the Form: Ensure you sign in two designated areas:
Signature matching the DP.
Signature matching the RTA.
Key Details: Remember to fill in the DRN & DRF numbers on the form.
Additional Documentation: Alongside two copies of the filled Dematerialization Form, enclose:
A self-attested copy of your PAN card.
A self-attested Mutual fund statement.
Complete the Details: This includes the holder's name, quantity, script name, folio number, certificate number, distinctive number, etc.
