If your name has changed or needs to be corrected in your FYERS Demat account, you must complete an offline modification process.
You need to submit the required forms and supporting documents to FYERS by courier or post.
Documents required for name correction or change
Submit the following documents:
- Name Correction Form
- Updated PAN card copy — self-attested.
Address proof reflecting the updated name — self-attested. You can provide any applicable document, such as:
- Aadhaar Card
- Passport
- Driving Licence
- Voter ID
Handwritten request letter mentioning:
- The reason for the name change or correction.
- Your signature.
- KRA Form
Valid supporting document for the name change or correction, such as:
- Marriage Certificate
- Newspaper publication for the name change
- Other valid name-change proof
- Bank proof reflecting the updated name — mandatory for all new name correction or change requests.
- Do not mention any dates on the forms.
- All signatures must be handwritten. Do not use digitalised signatures.
- Avoid overwriting or striking out any information.
- If you make a correction or overwrite any detail, add your counter-signature next to the correction.
- Do not use whitener on any form or document.
Note:
- The name must always match the details available in the PAN/Income Tax database.
- The KRA form must be filled as per the PAN and address proof submitted.

What If...
| Scenario | Solution |

| Affidavit, Notary document, or Marriage Certificate is unavailable | Provide another valid supporting document for the name change or correction, such as a newspaper publication or other valid name-change proof. Spouse Aadhaar is not accepted as an alternative name-change proof. |
| Bank proof still reflects the old name | Update your bank records first and submit bank proof reflecting your updated name. Name-updated bank proof is mandatory for all new requests. |
| A mistake is made while filling out the form | Avoid overwriting or striking out details. If a correction is required, add your counter-signature next to the corrected information. Do not use whitener. |
| Digital signature is used on the forms | Digital signatures are not accepted. All required signatures must be handwritten. |
| Name correction request is submitted online | The request must be submitted offline by sending the required documents to FYERS through courier or post.
